STUDENTS: Please disregard this reminder if you have already sent your forms!
Students should complete and return a Student Health Information sheet along with an immunization record by July 1. Requirements for Chatham University immunizations prior to arrival include a meningitis vaccine (Menactra) or a signed meningitis waiver form, two MMR (Measle, Mumps, Rubella) vaccine dates, three Hepatitis B vaccine dates or a positive blood titre AND a PPD skin test for all international students within one year of arrival to campus (A chest x-ray report needs to be included if the skin test is positive regardless of BCG vaccine history).
