Residential Student Online Reservation Form and Survey
Deadline Monday, December 1
All incoming students with fewer than four (4) semesters at another college or university are required to live on campus. Students need to complete the residential student online reservation form and survey by December 1. Students who return the survey by this time will be notified of their fall housing assignments around December 10. Students who complete the Online Reservation Form and Survey after December 1 will be notified of housing assignments as soon as compatible roommates can be found (if applicable) and assignments are finalized. Students will be notified of their assignment by mail to their permanent address.

All first year students will be assigned roommates and should correspond with their roommate before their arrival to campus in August. Students requiring medical accommodation should contact the Office of Residence Life. Transfer students are strongly encouraged to live on campus, either with a roommate or in a single room. For more information about housing and residential life, contact Jessika Coltz, Director of Residence Life, at (412) 365-1518 or jcoltz@chatham.edu.
Spring Term Invoices
December
The Office of Student Accounts will mail estimated spring term invoices in December for all pre-registered students. All final transcripts should be mailed to the Office of Admissions prior to the start of spring classes. Please contact the Admissions Office with any questions regarding enrollment to Chatham University. Invoices will be in the student’s name and sent to their address on file. An informational flyer explaining various payment options and other important financial information will be enclosed with the invoice. Invoices will list a payment due date in which your student account must be paid in order to secure your classes and avoid a late financial clearance fee. The payment due date will generally be one week before the start of the semester. For additional information regarding billing and payments, please contact the Office of Student Accounts at (412) 365-2797 (studentaccounts@chatham.edu).
Health Information Forms
Deadline Thursday, January 1
The Admissions Office sends the following three forms to all new students: Student Health Information Form, Meningitis Vaccination Waiver Form, and Immunization Record. All three must be returned to the Office of Health Services before August 1st for the fall semester or January 1st for the spring semester. For more information about Health Information Forms and health services, please contact Joanne M. Kurtz, Director of Health Services, at (412)365-1238 (jkurtz@chatham.edu).

Any student who does not complete the waiver process will automatically be enrolled in the Chatham University Accident and Sickness Insurance Plan and the premium billed to their student account.
